• Right Click on the 3COM Modem icon in the system tray
• Select "Launch Control Center"

You should get the following screen:

• Type CTRL/T to get the "Tools" menu to appear
• Select "Tools", "Troubleshooting", and then "ADSL Information"

This should produce the following screen:
